01. Overview of Mindset Evolution Program
The Mindset Evolution Program, offered by Mindset Evolution, LLC, provides evidence-based mentoring and educational content to enhance resilience and promote mind-body-spirit balance—but it is strictly an educational program, not a clinical therapy service. It is designed for emotionally resilient individuals seeking to build self-awareness and practical skills, and is not a substitute for professional mental health care; anyone experiencing serious concerns (e.g., suicidal thoughts, self-harm, severe anxiety, depression, or trauma-related symptoms) must seek support from a licensed mental health professional. All participants complete a pre-screening process to confirm suitability, and by enrolling they acknowledge the program’s limitations and accept responsibility for obtaining appropriate clinical care when needed.
02. Structure Mindset Evolution
2.1. By offering a comprehensive range of tangible and specific deliverables, through our flexible easy to use interface online platform. You can ensure that our online program provides participants with valuable knowledge, practical tools, and ongoing support to effectively navigate the experience.
Modules:
Depending on which program there could be anywhere from 4-8 video modules, ranging from 15 -25 minutes each, combined with PDF exercises/homework. Variation depending on program and the length of each course.

- Written Content: Detailed summaries, insights, and exercises accompanying each video.
- Interactive Quizzes: Short quizzes to reinforce learning and assess understanding.

- Weekly Q & A groups
Mentoring Calls:
- Group Q & A Sessions: Regular weekly live group calls available for questions, experiences, and personalized guidance.
- Individual Mentoring Sessions: Optional one-on-one sessions for tailored support (separate fee associated).
- Recorded Replays: Recordings of all calls for convenient access and review.
Resources:
- Reading Materials: Curated lists of books, articles, and research papers.
- Audio Recordings: Guided meditations, relaxation exercises, and affirmations for emotional well-being.
- Community Forum: Online discussion board for participants to connect, share, and support each other.
- Workbook: Comprehensive digital workbook compiling all module materials and exercises.

2.2. Since the entirety of the program is online and self-directed there are some potential risks with level of participation such as but not limited to lack of understanding the information, incomplete materials or exercises, lack of immediate support, decrease in motivation, technical issues, privacy and security concerns, overwhelm or burnout, inconsistency with self-reflection. Mindset Evolution, LLC has taken steps to address and mitigate these potential risks to ensure participants have a supportive and effective learning experience even when participating independently. By signing this agreement, you acknowledge and accept personal responsibility for your active participation in the Mindset Evolution Program. You agree to take ownership of your engagement in the program, understanding that the value and benefits you derive from it are directly related to the effort and commitment you invest.
2.3. Upon enrolment in the online program, participants will have immediate access to the initial content (when course starts). Subsequently, each new module will be released weekly on the same day. Participants will receive reminders when new modules are available. Additionally, reminders will be sent for upcoming mentoring calls or Q & A groups and other resources as they are made accessible.
2.4. Enhance your learning experience with personalized one-on-one mentoring calls. For an additional fee, these private sessions offer tailored support and guidance to address your specific needs and challenges. Work directly with our expert mentors to deepen your understanding, receive customized suggestions, and develop strategies for your unique situation. These sessions are designed to complement your online program, providing you with the extra support you need to achieve your goals.
Relationship with Mindset Evolution
2.5. Mindset Evolution is operated by Stacey Perko (or her facilitators, affiliates, employees, subsidiaries, licensors, licensees and other individuals may or may not be licensed professionals.
2.6. While Stacey Perko (or her facilitators, affiliates, employees, subsidiaries, licensors, licensees and other individuals may or may not be licensed professionals, the Mindset Evolution Program is not a psychology or psychotherapy service.
2.7. By consenting to participate in the Mindset Evolution Program, participants acknowledge that they are participating in a Mentoring Psychoeducational program and are not receiving psychological or psychotherapy services from Stacey Perko (or her facilitators, affiliates, employees, subsidiaries, licensors, licensees and other individuals may or may not be licensed professionals.
2.8. This form explains the different roles of Pinnacle Addiction & Mental Health Therapy and Mindset Evolution, clarifies what services you will receive from each, and ensures you understand and agree to their respective limits. It also protects our clinicians and educators by setting clear ethical and legal boundaries.
Definitions & Roles
- Pinnacle Addiction & Mental Health Therapy
- Licensed clinicians (LSUDC, QCSW, LCSW) provide individualized clinical therapy and mental health care.
- Governed by the NASW Code of Ethics and state licensing boards in Hawaii, Nevada, and Utah.
- Mindset Evolution Psychoeducational Courses
- Online, group-based Psychoeducational programs teaching skills (e.g., communication, stress management, triggers, boundary-setting, mind-body-spirit practices).
- Not clinical therapy; no diagnosis, treatment planning, or emergency services.
Acknowledgement of Services
I understand that:
- Therapy Services with Pinnacle Addiction & Mental Health Therapy are clinical, confidential, and regulated by professional licensing and ethics codes.
- Mindset Evolution Courses are educational only: they do not replace therapy, crisis intervention, or medical treatment.
- I am responsible for seeking additional professional care (medical, psychiatric, or emergency services) when needed.
Limits of Liability
By signing below, I agree that:
- Pinnacle Addiction & Mental Health Therapy and Mindset Evolution, their clinicians, educators, employees, and contractors are not responsible for outcomes arising from my use of psychoeducational courses alone.
- I will NOT hold Pinnacle Addiction & Mental Health Therapy or Mindset Evolution liable for any indirect, incidental, or consequential damages related to course participation or application of course content.
- If I choose not to follow a clinician’s or course instructor’s suggestions, that is my personal decision and responsibility.
Compliance with Ethical & Licensing Standards
- Pinnacle Addiction & Mental Health Therapy clinicians comply with the NASW Code of Ethics and all applicable state regulations (HI, NV, UT).
- Mindset Evolution adheres to educational best practices but is not a licensed therapeutic service.
Confidentiality
- Therapy sessions with Pinnacle Addiction & Mental Health Therapy are confidential, subject to legal limits (e.g., harm to self/others, abuse reporting).
- Course participation in Mindset Evolution may include group discussion; confidentiality in that context is respected but not guaranteed by professional privilege.
To avoid any potential conflicts of interest arising, current clients of Stacey Perko or affiliates, subsidiaries, and licensors, will accept participants on a case-by-case basis in the Mindset Evolution program.
2.9. Similarly, participants in the Mindset Evolution program can be accepted as clients of Stacey Perko or affiliates, subsidiaries, and licensors on a case-by-case basis.
2.10. Any requests for cross-referral or registration between current or former clients of Stacey Perko and Mindset Evolution LLC will be assessed on a case-by-case basis, and will only be accepted with the utmost ethical discretion, depending on the individual circumstances.
2.11. If in the course of participating in the Mindset Evolution program, issues arise which in “professional opinion of persons representing Mindset Evolution LLC” are beyond the scope of the mentoring relationship, a participant may be advised to see their General Practitioner and obtain a referral to consult with an appropriate clinician.
2.12. Mindset Evolution LLC may at any time and in their sole discretion, terminate a participant’s registration in the Mindset Evolution Program if the company considers it appropriate to do so. In the event of termination of a participant’s registration in the Mindset Evolution program on this basis, a partial refund of the participant’s registration fees MAY be provided, but NOT guaranteed.
03. Privacy and Disclosure of Personal Information
3.1. As part of your participation in the Mindset Evolution Program, Mindset Evolution LLC needs to collect and record information from you.
3.2. We enclosed a copy of our Privacy Policy, which we request you read and acknowledge your understanding of, prior to signing this document.
04. Technology Requirements
4.1. Mindset Evolution LLC is provided online through our website portal and by videoconferencing. You are responsible for the costs associated with setting up the technology needed so you can access online content and participate in video conferencing workshops.
4.2. To access Mindset Evolution LLC content, you will need access to a computer, video camera, speakers, and a quiet private space.
4.3. The privacy of any form of communication via the internet is potentially vulnerable and limited by the security of the technology used.
05. Conditions of Purchase and Intellectual Property
5.1. Mindset Evolution LLC’S website, its Mindset Evolution Program and all content provided within, is the intellectual property of Mindset Evolution LLC.
5.2. The reproduction, exploitation, redistribution or use of all or part of any visual, auditory or software element or document from Mindset Evolution LLC’S Website or the Mindset Evolution Program is prohibited.
06. Fees
Fees and Payment Terms
The cost of enrolling in the Mindset Evolution Program may vary based on individual circumstances, promotional offers, partial scholarships, or selected payment plans. All fees, including any discounts, scholarships, or special pricing, will be clearly communicated to participants prior to enrollment. By enrolling in the program, you agree to the following payment terms:
6.1. Standard Program Fee: The standard fee for the Mindset Evolution Program will be communicated during the registration process and is subject to change based on the chosen program in which you are enrolling and payment options available.
6.2. Partial scholarships: Partial scholarships may be available to eligible participants based on financial need, merit, or other criteria set by Mindset Evolution, LLC. Scholarship recipients are required to pay an upfront fee to hold their spot and any remaining balance by the specified due date to maintain access to the program as stated in the Partial Payment Plan Agreement that will need to be signed if you qualify.
6.3. Payment Methods: Fees can be paid via accepted payment methods, such as all major credit cards, Google Pay, and Apple Pay. Payment details and instructions will be provided upon enrollment.
By proceeding with the enrollment, you agree to these fees and payment terms as set forth by Mindset Evolution, LLC. It is your responsibility to understand and fulfill your financial commitments to ensure uninterrupted access to the program and that your account is in good standing.
07. Cancellation & Refund Policy
7.1. Mindset Evolution, LLC will grant a refund of the registration fee paid by any participant, within 24 hours prior to the commencement date of the Mindset Evolution program (or once you’ve accessed any module content—no refunds will be issued.). Please note that all payments, including those made under a scholarship or payment plan, are subject to our refund policy. Details of the refund policy will be provided at the time of enrollment and may vary based on the specific terms of your enrollment agreement.
7.2. Pre-booked 1:1 private mentoring call session may be canceled by the participant up to 24 hours prior to the session time without penalty, but must be re-booked before the end 60 days of original purchase of the Mindset Evolution program. No refund will be offered in respect of expired or unused 1:1 mentoring session.
08. Consent to participation in the Mindset Evolution Program
8.1. I confirm that I have read and understood the information in this document about Mindset Evolution Program and I acknowledge the following:
8.1.1. Mindset Evolution LLC is a psychoeducational mentoring service. Although the Mindset Evolution program by Stacey Perko (affiliates, subsidiaries, and licensors and other licensed professionals) is a registered clinical social worker, this is not a psychotherapy service and I will not be receiving psychotherapy services, treatment, or therapy in the course of my participating in the Mindset Evolution program;
8.1.2. I have read and understood the Mindset Evolution Privacy Policy.
09. Indemnification.
9.1 I hereby agree to protect, indemnify, save, defend, and hold harmless Mindset Evolution, LLC, its owners, employees and agents, from and against any and all liability, loss, injury, damage, actions, causes of action, damage to personal property, costs or expenses whatsoever (including reasonable attorneys’ fees and expenses) and any and all claims, suits and judgments which Mindset Evolution, LLC or its owners, employees or agents may suffer, as a result of or with respect to my participation in the Mindset Evolution Program. I agree that the maximum liability of Mindset Evolution, LLC or any of its owners, employees or agents to Participant and any other person, firm or company arising out of or in the connection with this Participant’s participation in the Mindset Evolution Program shall in no case exceed the amount Participant paid to Mindset Evolution, LLC during the twelve (12) months prior to such claim.
